CLCM is the fixed point of entry for individuals, children and their families who are in need of support for intellectual and developmental disability services. CLCM operates programs from Josephine, Curry, Coos, Douglas, and Mid-Columbia (Hood River Wasco and Sherman) Counties.

The CDDP program assists with the following:

  • Determination and documents eligibility for services and funding
  • Choice Counseling and assistance with obtaining general case management services
  • Assesses the individuals needed supports and desired services
  • Assists the individual and their family with obtaining services
  • Coordinates an Individual Service Plan for 24 hour residential services, In-Home Comprehensive, Employment or Community Living Support (CLS alternatives to Employment) services, medical care and supports, and other supports required
  • Monitors the provision of services given from Comprehensive providers of Residential, Employment and Community Living Services, and monitors the quality of services provided for individuals
  • Provides consultation to providers and families
  • Licensing of adult and children’s foster homes
  • Protective Services and investigation for adults
