All requests and referrals to residential services are generated from CLCM to each provider.
24-hour Residential Services consist of a variety of provider organizations that operate homes within the community. All 24- hour licensed homes are staffed 24 hours per day with trained staff to support individual’s care and support needs. Although programs vary, services will include medical, clinical, social and personal care support needs. All homes are licensed by the State of Oregon and services provided are monitored by CLCM service coordinators.
Supported Living Services are residential services that can include supports in a family home or persons own home/apartment. Services are person centered/self-directed supports. Supports can include money management, medical/ medication management, community integration, personal care and daily living skills.
